Insulin has a complicated relationship with the functioning of the ovary, and women with poorly controlled diabetes often have fertility problems as well. These problems include infrequent ovulation (and hence difficulty getting pregnant), recurrent miscarriages and congenital abnormalities. Thankfully, all these problems seem to resolve if the diabetes is well controlled, and in modern times diabetics have problems only infrequently.
